PRP FOR ANTIAGEING

Introduction –
PRP is abundant platelet (approx 4-7 times the mean platelet count) in a small volume of plasma. PRP is a revolutionary breakthrough in the stimulation and acceleration of soft tissue healing and rejuvenation.
Indication –
Androgenic Alopecia skin Rejuvenation, Acne scars, wound ulcers, Striae distensae.
Contraindication –
Critical thrombocy to penia Hypofibnnogenemia
Haemodynamic instability
Sepsis
Acute and chronic infection
Chronic liver disease
Anti Coagulation therapy
(Warfarin, Heparin)
Mechanism of Action –
PRP increases proliferation of dermal tibroblast, increases MMp-1, MMP-3 and removal of damaged extra cellular matrix Neocollagenesis due to procollagen type-1 production Accelerated wound healing due to increase expression of G1 cell cycle regulators.
Pre Procedure :-
Avoid NSAID 1 week prior to procedure.
Procedure :-
Apply Numbering cream under occlusion wait for 30 min. once the Area to be treated is numb. Inject PRP Intradermally, subdermal, by insulin syringe on the face as illustrated in the figure below.
Repeat the injection monthly or at a gap of 6 month and maintenance 1/year.
PRP Extraction –
13.5 ml. of blood is withdrawn from patient in 20 ml. syringe with 18 G needle which is prefilled with 1.5 ml. of ACD. Insert 18 G needle in the side hole in the PRP kit and press the plunger of the syringe so that blood is filled in the PRP kit up to the mark as illustrated in the image below.
Centrituge the PRP kit at 3500 RPM for 4 min. We can see 3 layers in the PRP kit Platelet poor Plasma Platelet Rich Plasma Blood layer
Aspirate the PRP from the centre hole in the kit with 18 no. Needle attached to 1 ml. syringe just above the blood layer and the buffy coat comprises of 4 ml. of PRP
Activation of PRP can be done by addition of calcium chloride iml of Cacl2 every 10 ml. of PRP or PRP gets activated by mechanical trauma collagen is also an endogernous activator of PRP.
Post Procedure –
Avoid massage for 2 days and peels or ablative lasers on face for 3-4 week.
Conclusion –
PRP in dermatology appears premising and good results are seen when PRP is used for rejuvenation and Anti-ageing but long term studies is required with larger number of patient and there should be a consensus regarding optimal method of preparation, procedure standardization is required.
